How to Complete Forms 1. Print a copy of the following forms to use as worksheets. . Personal Information . Activities & Interests . Community Service . Family Information . Favorites Information . High School Information . Honors & Awards . Talent Introduction 2. The forms listed above will be available on the Junior Miss website as interactive PDF files. These forms should be filled out on your computer and printed out for you to turn in. The judges will use these forms during interview and should be letter perfect. Check spelling, grammar, and typing for errors. Two (2) copies should be made (not stapled, no two-sided, not folded) and returned to April Bragg, local program director. 3. Make sure you include the name you prefer to be called. 4. DO NOT LEAVE ANY BLANKS. The judges need some information from you in order to get an idea of what they would like to ask you during interview. 5. Be sure that you keep a copy of the completed forms for yourself for review before your interview. 6. What the judges see on these forms represents you, so be sure to check and double-check grammar and typing errors!! 7. Each form has a blank for (Local) and (Program Year). Fill out as shown below: Local: Milledgeville-Baldwin’s Junior Miss Program Year: 2008 Even though the competition takes place in 2007 and you begin the program in 2006, this is the 2008 program year because it is based on the year you graduate high school. NO EXTRA PAGES WILL BE ACCEPTED WITH THESE FORMS. ANY ADDITIONAL PAGES WILL BE DISCARDED.
